Medical Cannabis Evaluations
Certified Medical Marijuana Physicians helping patients get access to a safer alternative treatment and reduce harmful pharmaceuticals. Contact Canna Alternative Care and a team member will help to schedule an appointment for an evaluation and answer any of your questions. Must be a Florida Resident and patients seen by appointment only.
initial Evaluation $150
During the initial visit the physician will review your medical history, present symptoms, and perform a physical examination. The physician will discuss the recommendations and treatment plan. Your personal information and dosage recommendations will be placed into the Registry. Once approved by the Registry, you will be ready to purchase your medications from the licensed dispensaries.
Recertifications $150
As required by the state of Florida, a physician must perform an evaluation to reassess the patient every 7 months. This visit can be done in person or via telemedicine. During this visit, the physician will discuss the current regimen, symptoms and current treatment program and perform a physical exam. This visit covers ordering for the next 7 months. Any adjustments within the 7 month certification, can be done over the phone, at no charge.
State Application Completion Fee is $25
